30 year old male patient with diarrhea

 30 year old male patient from chityala came to the opd with chief complaints of recurrent loose stools and abdominal pain on 3/01/ 23.  HOPI - The patient was asymptomatic on 2/ 01/ 23. He had a days old juice after which he had developed recurrent lose stools with a vomiting in the late evening.The stools had low volume and increased consistency. This was followed by abdominal pain which was sqeezing type in the umbilical region. There was no radiating pain. His pain did not reduce on any food intake or any other home remedy.   Past history - none  Personal history - The patient is not a known case of  -DM  -Asthma  -HTN - Epilepsy  -TB -Thyroid  Family history - none  Drug history - The patient had administered an ELDOPER TAB before coming to the hospital which didn't reduce any pain . Allergic history - none  Investigations -  Diagnosis - Acute gastroenteritis secondary to food poisoning.  

55 year old male with Cerebrovascular accident

 55 year old male a goat Heder from Gouraram came to the department on 23/ 12 / 2022 with chief complaints of weakness in left upper arm associated with sudden involuntary moments.  HOPI - Patient was asymptomatic 10 days back. On 17/ 12/ 22 he developed sudden involuntary moments on the left upper limb associated with weakness and loss of speech. The patient was taken to a hospital in nalgonda where he was admitted and the following tests were done-  -MRI brain -CBP -Echocardiogram  - Lipid profile  - CT cerebral angiogram  - ECG -LFT - CT neck angiogram  -Renal function test  The following medication was prescribed-  - Metformin hydrochloride sustained release tablets ip - 500mg - Atorvastatin tablets ip- 40mg - pantopazole gastroresistant - 40mg and domperidone prolonged release 30mg gelatin capsule. - clopidogrel - 75mg and aspirin - 150mg capsules  - Cefixime and Lactic acid bascillus dispersable tablets  - Telmisartan tablets - 40mg  - soluble insulin injection 40 IU/ ml  - cereb

35 year old male with Diabetic Nephropathy

CHIEF COMPLAINT - A 35 year old male patient resident of choutuppal and driver by occupation presented to the department with chief complaints of fever along with pain and swelling with pus on the sole of the right foot near the second toe since 10 days.  HOPI - The patient was apparently asymptomatic 10 days ago. He developed fever which was continuous and along with it he also noticed a swelling on the plantar surface of right foot near his 2nd toe associated with pus.He also had pedal edema on right leg. He visited a RMP ten days ago and was prescribed medications which reduced his fever for a while but did not reduce the swelling. After 3 days he visited the department as his fever didn't reduce and the swelling and the pus oozing had increased. He had no history of night sweats. PAST HISTORY -  Patient is hypertensive since 1 year  Diabetic since 11 years ( initially he used tablets to control diabetes but since 4 years  he is kept on insulin therapy )  Pedal edema since 10 da

29 year old female with URTI

A 29 year old female, a resident of katangur , laborer by occupation presented to the department with chief complaints of fever and productive cough. HOPI-  The patient had - productive cough since 10 days associated with cold and was not associated with throat pain. The patient came to the department on 12th December 2022 with the same complaints and was asked to get admitted after a chest X- ray and was prescribed some medications pertaining to the symptoms. But the patient did not get admitted and continued the medications for a week.Then she came to the department on 19th December 2022 with complaints of fever since 5 days and headache since 4 days and also complained that cough and cold did not completely subside.Fever was reducing after paracetemol admisnistration, but was again relapsing after a few hours.
